The team help those recently discharged from hospital to stay safe and well for up to six weeks after your time in hospital.

They can support you in your home and complete a personal health and wellbeing assessment and a home safety check to identify risks in your home.

Additional services include: a basic shop and advice on eating well and meal preparation.

They can offer advice about local social activities and groups and also support you to attend a group including those run by Age Friendly Salford or at Critchley Community Hub in Swinton.
