Cardiovascular Rehabilitation Team

The Cardiovascular Rehabilitation service in Salford provides a city-wide, multi-modal, person-centred service to patients and their families/carers.

The service supports people and their families to return to a full and active lifestyle as soon as possible after a cardiac event, surgery or a diagnosis of cardiovascular disease. In helping them to understand and self-manage their condition, it promotes positive lifestyle change, which takes into account social, physical and mental wellbeing.

Referral Details

You can be referred to the service if you are over 18 years old and have experienced one of the following within the last 12 months.

  • you have been diagnosed with a heart attack

  • coronary stent inserted 

  • coronary artery bypass graft surgery

  • heart valve replacement or surgery

  • congenital heart surgery repair

  • heart transplant

  • implantable cardioverter defibrillator (ICD) inserted

  • biventricular pacemaker inserted

  • you have been diagnosed with stable angina

  • you have been diagnosed with stable heart failure

  • you have been diagnosed with peripheral arterial disease (PAD)

If you are an inpatient at Salford Royal and you have had a heart attack and/or angioplasty/ stent then you will be referred by the ward staff. The cardiovascular rehabilitation team will aim to see you before you go home.

If you live in Salford and you have had heart surgery or a heart attack at another hospital then you will be referred by them to the service. 

You can be referred by your heart or vascular consultant or specialist nurse/podiatrist.

Your GP can also refer you to the service. There is a referral form available to your GP on their computer system.
