Salford Community Diabetes Young Persons Service
The Salford Young Persons Diabetes Team works closely with young people with diabetes and their families by providing education and support that is age appropriate, up to date and consistent.
The service is provided for young people with a diagnosis of Type 1 or Type 2 diabetes from 18 years old.
Patients with Type 2 diabetes will need a referral from their GP for a 1-1 clinic appointment but can self-refer for education.
Patients with Type 1 diabetes can self-refer for both 1-1 clinic appointments and education.
Diabetes specialist nurses are available to answer any urgent queries from 9:30am to 4:00pm, Monday to Friday.
