Salford Adults Community Diabetes Team
The Salford Adult diabetes team is provided for patients aged 18 and over with a diagnosis of diabetes, offering 1-1 support with clinic appointments and various group workshops.
Patients with Type 2 diabetes will need a referral from their GP for a clinic appointment but can self-refer for any of our group workshops.
Patients with Type 1 diabetes can self-refer for a clinic appointment and education.
Specialist nurses are on call to answer any urgent queries from 9:30am to 4:00pm, Monday to Friday.
