What is a Early Support?
Early Support is a programme designed to deliver better co-ordinated services for parents/carers of disabled children.
The Early Support Key worker is the first point of contact for families.
The aim of key working is to ensure the provision of holistic care and support to the family to meet the individual requirements and ensure aspirations of the child, and their family are met.
The Key worker will visit the family at home and also support at meetings.
They will also ensure information is shared so families don’t have to keep repeating “their story”.
Who is the Service for?
A family whose child meets the following criteria:
Has multiple health needs or a disability
Has 4 or more services involved that are not universal
The Early support key worker will:
Support the family in completion of the introducing ourselves document.
Keep in regular contact, as well as responding to parents concerns.
Provide accurate and appropriate information and signpost to relevant services.
Listen to parents/carers views and ensure that other professionals are aware of them.
Act as an advocate during meetings.
Provide an effective link between parents/carers and those services working with their child.
Provide emotional support to families.
Offer families Early Support information and materials.
Coordinates multi-agency Service Planning Meetings with the family at the centre.
Provide transition support in a timely manner.
Support families in identification of settings.
Support families in the completion of the EHC process as needed
Ensure parents are made aware of their choices.

A referral can be made by any agency, health, education, social care or families themselves.
